You may request the deletion of your personally identifiable information (PII) and the deletion of your account from Leva Clinic. However, as a health clinic, certain information, such as your health records, cannot be deleted. For detailed information on data deletion and retention, please refer to our Privacy Policy.

To initiate a data and account removal request, please send an email to patients@levaclinic.com with the subject line "Data Removal Request." Include your name, date of birth, and address in the email. If additional information is required to verify your identity, we will request it. Your request will be processed within 30 days.

Upon completion of your request, you will be notified accordingly.
